宏济堂始于1907年，由北京同仁堂资东、电视剧《大宅门》主人公原型乐镜宇先生创办。乐镜宇系同仁堂十二代传人，1902年捐官山东候补道由京来济候补，期间，承家传岐黄之术，研习阿胶制作技艺；1906年，受山东巡抚杨士骧委托，创办山东官药局出任总办；1907年，斥巨资还官银获山东官药局承授权，立堂旨“宏仁广布、济世养生”，故更名宏济堂。宏济堂得同仁堂之滋、遵同仁堂故事，穷究方术，精研修合，以药材道地、秘方效验、名医坐堂、工良艺湛、药真价实、童叟无欺独创风格。

1909年，宏济堂于济南府西门外趵突泉畔——中国阿胶圣地东流水街建阿胶厂，同年所产东流水阿胶进奉皇室被御赐为“九天贡胶”；1915年，宏济堂东流水阿胶获巴拿马万国商品博览会金奖；20世纪10－30年代，宏济堂北京建参茸阿胶庄，济南建人文神韵之东、中、西号三店堂，开国药同城商铺连营之滥觞。

宏济堂历经百年，遵堂旨、承祖训，施粥赈灾，捐资助学，抵制日货，正义爱国。倾其力、解囊创办山东国医学堂；尽其能、成就培养一代山东名医；冲围困、贸易于抗日根据地；其“声誉、药品，推及黄河、长江两域，无间妇孺，咸使间知……”与北京同仁堂、杭州胡庆余堂并誉为中华三大名堂。

解放后，已是乐家总掌门的乐镜宇年逾八旬，商号铺务逐交子孙辈接管，作为中国民族工商业的优秀代表，受党和政府之邀，由继任者其侄乐松生先生代其二次荣获毛主席、周总理之接见。宏济堂则公私合营，直到发展为现在的山东宏济堂医药集团。
